WE PARTICIPATE with many insurance plans, including:
(Aetna, Anthem BCBS & FEP, Connecticare, Harvard Pilgrim, Medicare, Multiplan, Tri Care Prime, and UHC/Oxford.)
We are NO longer accepting NEW Medicaid patients. If you are a Medicaid patient and have NOT been seen in the office within three years you would be considered a new patient.
If you have any questions about our participation with your insurance plan, please call our office or your insurance company before scheduling an appointment.
If your plan requires a Referral to be seen by a Specialist, please obtain it from your Primary Doctor before your visit.
Our online scheduler allows patients to self-schedule up to 90 days into the future. If you are an established patient, you should see a “welcome back!” notice appear after validating your name and date of birth. If you do not see this even though you have been a patient of the practice, please call the office after scheduling your appointment to verify your information, before filling out the registration form which will be e-mailed to you.
Registration
After scheduling an appointment, you will receive a registration link from eDerm.
Login with the patient’s last name and date of birth. If you didn’t receive the email, please check your Spam Folder. If you still can’t find the email or can’t login, please call the office.
